UNGAA, Inc. Distinguished Professor Award Nomination Form

Use this form to nominate someone for the UNGAA, Inc. Distinguished Professor Award. A maximum of two recipients per year will be chosen. One from UNG North (Dahlonega and Blue Ridge) and one from UNG South (Gainesville, Cumming, and Oconee). *BEST BROWSERS FOR USE ARE EITHER CHROME OR FIREFOX.

The University of North Georgia Alumni Association Awards and Scholarship Committee is seeking nominations for the UNGAA Distinguished Professor Award. Nominations need to be completed by December 15 each year. Nominations received after December 15 will not be considered until the following academic year. Nominations from across all UNG campuses are encouraged.

Please consider the following when completing the form.

-Prepare the nomination assuming that the selection committee does not know the nominee.

-Only this form and the nominee's curriculum vitae (not to exceed five pages) will be considered.

-You are encouraged to seek assistance from the nominee and/or the nominee's department head.
