Access lists are automatically PURGED at the conclusion of each academic term. Access lists will need to be resubmitted at the start of each new semester.
By submitting this form, I acknowledge the following: (i) the student(s) I have listed on this form is(are) currently enrolled in a class that I teach and such student(s) need(s) access to the building and room number listed for class-related purposes; (ii) I have reminded the student(s) that after-hours access, if granted, is only for class-related purposes and only for the building and room specified; (iii) I have informed the student(s) that any after-hours access granted pursuant to this request may be revoked at any time by the University in the University’s sole discretion; and (iv) any after-hours access granted pursuant to this request will expire at the conclusion of the current academic term and will require resubmission for any subsequent term.
