Once this form has been received, and approved, you will be registered for the appropriate applied lesson by the Music Department. Two-credit lessons are only available for the following students: - Declared Music Minors (this will be verified in Banner) - Music and Music Education Majors who are on provisional admission to the program All other students are eligible for one-credit study only and may self-register once they have auditioned and been given permission to enroll. *** Please note that new students should speak with their desired applied instructor prior to filling-out this form to audition and discuss availability in the studio.
